Follow the instructions contained within the 'SIA Licence Application Form Guidance Notes' booklet (download size: 803kb), which we will send to you along with your application form. You should fill the form in using BLUE or BLACK ink.

• In the 'Address History' section of the form, the current address must be shown as:

• The date below this should be the month and year of application (e.g. if applying in January 2008: 01 2008).
• Your previous address details should show your permanent residential address overseas.
• A full five year address history should be provided so please use the continuation sheet if necessary.

Supporting Documentation

Information on the type of supporting documents we require can be found in the guidance notes.

In some cases it is impractical or illegal for passports to be posted outside of their country of issue. Consequently, we will accept a notarised copy of a passport from a suitably accredited notary in the country in which you reside. All other documents must be originals.

Documents will be accepted as long as their contents are clear or they are supported by a translation into English by an accredited translator (for example: a bank statement must show your name and address and be identifiable as a bank statement).

Sending In your Application

1. Check that the form is correctly and fully completed - especially the five year address history.
2. Check that the form is signed and dated
3. Check that the photograph is counter-signed correctly (i.e. signed and dated by the counter-signatory)
4. Check that payment is included
5. Check that all supporting documents, criminality check certificate and translations are included. If you are sending a notarised copy of your passport please attach a cover note to the front of your application stating that you are an "overseas applicant with notarised passport".
